For established patients, other times can be arranged with Dr. Alvarez to accommodate special needs such as urgent care visits. So, if you do not see a time that will accommodate your needs and you are established with Dr. Alvarez, please call us and we will certainly try to work you in or assist you in any way we can.
HOW TO SCHEDULE:
PLEASE NOTE – for New and Established patients – due to limited staff, we ask that if you are able, schedule your appointment online/ via the PORTAL (once you have an account with us).
**Once you have an appointment, up to ONE WEEK BEFORE your scheduled visit, you will see a ” CHECK IN NOW” option once you are logged into the portal.
PLEASE do the CHECK IN NOW – This allows you to confirm insurance, demographic information and to ENTER/ UPDATe or Change your health information including medications. There is also an option to pay your copay as well. THIS STEP helps our micropractice model, limited administrative tasks at the front desk, and will enhance your experience by having more face to face time with your provider!
To schedule your appointment please follow the instructions via the new PATIENT PORTAL. **PLEASE NOTE, if you are scheduling a SAME DAY appointment, PHONE us to notify us as well, this will ensure your appointment has been confirmed.
TO CANCEL an appointment you have made – Please follow the procedures on the portal. ***Please note there will be a fee for same day appointments cancelled without a 24 hr notice by phone/voicemail.
